Fax Diagnostic Report
Fax front and back of insurance card
Fax prescription for ABA from primary doctor

Fax any previous speech assessments or notes (or sign consent to release records)

We are a traditional provider for the NOW/COMP waiver with DBHDD in the services of Speech and Behavioral Consultation in region 6.
